Disability Van Service is available to employees, students and visitors with temporary or permanent mobility impairments. No fare is required to use the Disability Van Service; however, current medical documentation and advance reservations are necessary. (See "Authoriztion Process" section below for more information.)

Authorization Process
TAPS requires students to have a doctor's note as medical justification for use of the Disability Van Service (DVS). UCSC students can get medical justification, if warranted, at the Student Health Services (SHS) and can request transportation by the DVS to the SHS for the initial evaluation. Students with UCSHIP insurance or with the CruzCare access plan can be seen at the SHS for a visit for illness or injury without any fees. Additionally, students without campus insurance can be seen by the triage nurse, who may be able to provide medical justification, without any fees. Fees would be incurred at the SHS for students without campus insurance if there is a need for further evaluation by clinicians at the SHS.

If you need additional information about authorization procedures for the Disability Van Service give us a call at 831/459–2829 or send an email inquiry. Medical documentation must be provided as follows:

Students with permanent disabilities should contact the Disability Resource Center at 831/459–2089 to learn more about obtaining authorization.

Students with temporary disabilities should contact the Student Health Center at 831/459–2211 to schedule an appointment to obtain authorization.

UCSC employees can fax a letter from their doctor or medical practitioner (temporary conditions must include an end date) to 831/459-4234.
